Alcohol & Other Drugs (AOD) Assessment and Care Planning Clinician

The AOD Assessment and Care Planning Clinician position provides for a dedicated assessment and treatment co-ordination role supporting joined up services for people in Sunbury and surrounding areas wishing to address their use of methamphetamine or other substances.

The successful candidate will be suitably qualified and experienced in the provision of assessment, counselling and case management services preferably with AOD or dual diagnosis. You will also have demonstrated experience in therapeutic approaches such as: Motivational Enhancement Therapy, Cognitive Behavioural Coping Skills Training, Family Inclusive Practice, self-help groups, Community Reinforcement Approaches.  Your proven ability to effectively work with people and families experiencing problematic alcohol and other drugs use in a respectful, compassionate and positive manner will help you be successful in this position.

Sunbury Community Health is seeking expressions of interest for Family Services Case Managers. We have several positions available and hours will be negotiated with successful candidates.

This position is delivering Integrated Family Services as part of a small team, within the context of a wider Child FIRST alliance covering Hume and Moreland Local Government Areas. This position requires an energetic and skilled practitioner who will find motivation in working with families to improve the lives of children.

The successful applicant will find satisfaction in working as part of a team, and working within a case-management framework  to address multiple and complex needs of families, in a supportive and child-focussed way.

This position requires relevant tertiary qualification in Social Work or equivalent. It also requires a current driver’s license, police check and Working with Children Check.

Family Violence Men’s Case Worker (Male Worker)

The Family Violence Men’s Case Worker position plays a vital role in the delivery of services provided to increase the safety of women and children and change the behaviour of men who use violence against family members. The position works collaboratively in a team based environment to engage and provide services to men using violence and provide support to their family members.

The successful applicant will possess a tertiary qualification in Social Work (or relevant discipline) and ideally, a No To Violence (NTV) Graduate Certificate.  Most importantly, you will have a demonstrated understanding of the social and gendered context of family violence and its impact on women and children and a successful track record of supporting men to change their lives and those of their families.

Sunbury Community Health and the Hume Whittlesea Primary Care Partnership are seeking to recruit a Diversity Advisor.  Based in Broadmeadows and working across Melbourne’s North Metro Region, the Diversity Advisor will facilitate and drive diversity planning and practice across HACC Program for Younger People and Commonwealth Home Support Program funded organisations.   The Diversity Advisor position is part of the North Metro Sector Development team and will work with relevant networks and alliances to promote diversity planning and providing support to funded organisations in the development and implementation of their diversity plans.

To be successful in this role you will have a tertiary qualification in a relevant discipline (project management, communications, training or health) and experience working in the health, aged care or community sector.
