Casual Roles – Activity Groups Support and Respite Care 

Do you have aged care or lifestyle training and looking for casual work? 

We’re always looking to employ enthusiastic people to support a variety of activities in group settings, primarily for older people, and people who can support individuals to go out and about in the community (providing respite to their carers).

Hours of work can vary from 3 hours per week to 3 days on a monthly roster, for successful candidates. Hours are between 8.30 am – 5 pm weekdays.

Care Manager – Home Care Packages

0.8 EFT (negotiable)

Our Health and Wellbeing Unit is seeking a Care Manager in our Home Care Package program. We are looking for an experienced and hard – working  aged care professional committed to promoting consumer directed care, supporting people to stay at home.

Home Care Packages are allocated to eligible people by the Commonwealth Government to support people, generally 65 years of age or over, who wish to stay at home. The Care Manager assists a Home Care Package holder to make choices around their care needs, develops a care plan and is available to coordinate the required range of supports within an allocated budget.

Applicants must have a tertiary qualification in Allied Health, Social Work or Nursing and have experience in goal directed care planning and coordination of services using the principles of consumer directed care.

NDIS Liaison Officer

0.6 to 0.8 EFT (negotiable)

Our Health and Wellbeing Unit is seeking an NDIS Liaison Officer to join the Care Coordination team at Sunbury Community Health. This position will provide administrative and program support and a point of engagement for community members who will access Sunbury Community Health services using National Disability Insurance Scheme (NDIS).

Applicants with a Diploma in Community Services or a related field, and knowledge and experience of the disability sector are encouraged to apply.
