Support to live your way, in your home.

A Home Care Package is a program of the Australian Government that provides money to buy services for eligible older people to live well and safely at home and in the community.

Sunbury Community Health can organise help with daily living and healthcare as part of a home care package, including:

  • activity/social groups
  • occupational therapy
  • physiotherapy
  • podiatry
  • exercise groups
  • help to shower
  • transport
  • help with house work
  • shopping
  • gardening
  • home maintenance

and many other types of help.

With a Home Care Package, you have choice over what services you get, where and when you get them and who you get them from.

We can help you to manage your Home Care Package funds, and find and organise the services you need to keep living the way you like – at home.
